Narrative

While working to remove a sheave wheel from its spindle on the 1st floor of the Prep Plant, the wheel broke free and caused the employee to quickly move out of the way. Employee struck the edge of his left foot against the concrete base of an electric motor. He sustained a fracture to the 5th metatarsal of his left foot.
