Narrative

A crew of 4 men were installing a belt, using a forklift and boom truck. Two employees were watching. One stationed at the tail pulley; one between the tail pulley & boom truck. The Forklift was on the opposite side of the conveyor pulling the belt. As the belt went around the pulley the employee reached in. The belt jumped and pulled his hand into the tail pulley.
