A roof fall occurred in the 1 East Panel, #4 entry in the pillarline between crosscut #98 and crosscut #99 measuring 10ft. x 18ft. x 5ft. Five foot fully grouted roof bolts were used to support the area. An undetected slip and kidney bed was determined to be the cause of the accident. This area was mined in December 2005.
