Narrative

Employee was getting out of deck of scoop-sent was not bolted down securely-employee struck his chest on the left side against scoop frame when seat flipped over. Continued to work-chest continued to hurt him-went to doctor on 12-28-05-employee had fractured ribs & bruise. Continuing to work reg. Job
