## Narrative

Hopper beneath S3-L had filled with rock dust when the shaker's bottom conveyor failed apparently due to the failure of the convyor's v-belts.  The employee entered the hopper via the bottom or tail end to shovel out the plugged hopper.  The material shifted inside causing the conveyor to move pinching the employee between spilled material and a hopper cross-brace.
