Narrative

When pouring oil from 5 gallon buckets into fct, experienced pain and discomfort to his right shoulder.The pain continued and on 9-6-05 received initial medical treatment for the alleged injury. An mri was taken indicating a torn rotor cuff. EE continued to work all scheduled days unti 1/3/06. on that date started missing scheduled work days.
