## Narrative

Employee was lifting a block of sandstone using a crowbar when the stone broke in half. This caused him to lose his footing and fall to the ground twisting his lower back. Physician's diagnosis: low back strain. Employee will be off work 7 to 10 days. Next appt. 1-26-06.
