## Narrative

two men install bottom rollers on excavator frame. He use an 3/4"" air impact wrench with a 6"" extension,swivel attachment to snug roller bolts. He hold air wrench in an inverted position from  kneeling position making socket at about height. While tightening one of the bolts the socket slipped off bolt head then came off the swivel attachment and struck him in the face and mouth.
