## Narrative

EE suffered chemical burns to both eyes. Transported by air ambulance to Univ. of TX Medical Branch in Galveston. Treated & released 1/21. Incident occurred when R31 relay relief tank pressurized causing lid to separate from top of tank. Liquor from tank sprayed south & east of tank & contacted EE & 8 others working in area. EE & 1 co-worker lost work days.
