## Narrative

Employee stated that he was drilling the outside roof bolt hole in the No. 7 Room, when a piece of slate rock fell and struck the operator canopy and a piece measuring 2' x 2' x 1"" struck him on the lower right side of his back and knocked him forward into the controls. He stated that he did not observe any loose roof material when he had installed the inside roof bolt.
