Narrative

While employee was installing cable bolts and wire mesh, he had just installed a bolt and it still had tension on it when he lowered the boom of the twin boom bolter. When he did this, it caused the bolt wrench to fly off striking him in the left forehead just at his eyebrow. This resulted in a laceration of about 1"".
