On #1 panel off of #5 belt near spad # 1261 a roof fall was found on 1/26/2006. This area is traveled on a weekley exam. (Outby Area) No injuries occured and ventilation was not disturbed. Area dangered off and cribbed. Fall was 20' long X 20'wide X6' high.
