## Narrative

Employee was cleaning the waste dust scales using an air hose.  When he turned the air on, the hose popped off and the coupler struck him in the face.  Xray at time was negative for fracture, but ee later required surgery for deviated nasal septum. Underwent OP surgery on 2/1/06.
