Narrative

Employee was cleaning the #5 mill pedestal area. A 6"" X 12"" x 1/4"" piece of metal was removed from the grease and placed on the pedestal. The metal slid off the pedestal and struck the employee on the right foot resulting in a metatarsal fracture of the right foot.
