Narrative

Employee was dismounting a wheel loader. As he came off the last rung of the ladder, he stepped on a 3""+ rock, bruising his right foot. He did not request medical attention until 2/17/06. The physician returned him to work with restrictions after his follow up visit on 2/20/06. He has to use crutches and stay off foot. We can accommodate.
