Narrative

Employee had opened the door of a tool box that is mounted on the side of a flat bed pick-up. The truck was close to a berm and when the employee stepped on the berm his feet slipped and he fell into the door of the tool box. Original Xrays were negative. He went back on 02/21/06 and a cat scan showed two cracked ribs. This makes this reportable.
