Narrative

Employee standing directly under 2x2x1/4"" angle while millwrights fitting side walls of a duct collector screws housing when the angle fell 1' onto EE's hard hat. He was taken to the hospital for observation. He was released to full duty. During the night he went back to hospital & got restricted duty release. He resigned 2/24/06.
