Narrative

EE was shotcreting & realized that he had a hole in his boot. He made the choice to continue working and did not report it to anyone. That morning in the shower, he noticed a redness in his foot, but still did not report. He took a PTO day next day but did not go to doctor. On 2/22 was in hospital. On 2/23 employer was told.
