## Narrative

Alledged incident was to have occurred while this employee was using a plasma cutter.  He first though he got flash burned but later a piece of metal was found in his eye and removed.  Cause of this injury was not wearing goggles. Compliance with rules was a factor, proper protective equipment was a factor, miner proficiency a factor, mining equip/systems not a factor.
