Narrative

On 3/20/06, an unintentional roof fall occurred at the #6 crosscut in #5 entry on the conveyor beltline. The area was initially supported with 10' point anchor roof bolts, the fall measured 25' x 18' x 12' high and the roof fall will be cleaned up.
