## Narrative

I was unloading crib blocking from the Eimco scoop bucket by hand with a co-worker. A piece of head coal & slate 4"" x 2' x 3' fell and struck my hard hat and upper body. A small piece of this sharp slate hit my head after my hard hat was knocked off. This injury did not include any aspect of compliance with rules or regulations, job skills or attitude.
