Narrative

Employee was in a manlift vertically fitting a 2"" pipe that was 18"" long. Employee welded a tack and as he adjusted the pipe the tack broke and the pipe struck his upper lip. Employee recieved stitches and was released back to regular duty. All appropriate PPE was worn.
