Narrative

EE was to go to the 4A inlet duct to clean out material from the bottom of the duct. The men working at that location reported starting the cleanup at approx. 6:00 am. At approx 6:45 EE lost consciousness falling forward to the surface as he was sitting and shoveling material down the ducts slope.
