Narrative

Installing 6"" yellow mine water line. Put 2"" shut off valve on end. Filled line with mine water. Opening 2"" water shut off valve to purge line of rocks and debris. First coupler outby failed and jetted 6"" line inby. EE's hand was on valve causing hard jerk on shoulder.
