## Narrative

On 5/1/06, the carpenter crew was dismantling and removing a scaffold on the decanter structure.  Employee was handing a board down & felt a pain in his back and continued working. He reported to medical at 1:30 PM for treatment and returned to work.  On 5/10/06, he was seen by a doctor and was restricted to no lifting and no overhead work.
