Narrative

Employee reported: Installing a roof bolt in drilled hole.This was the rib bolt which required the addl curtain hook over the bolt. When rotating bolt to the roof, the curtain hook started to swival around, ee reached to straighten hook while still rotating and raising the bolt. Tip of right middle finger was caught under corner of pin plate causing laceration
