A roof fall occurred in the 14 North Panel in #3 entry in 1/2 the intersection of cross-cut #22 and extending through the intersection of cross-cut #23 measuring approximately 60ft. x 19ft. x 5ft. The area was supported by 5ft. fully grouted roof bolts. An undetected slip was determined to be the cause.
