Narrative

The employee was in the process of raising a dredge pipeline with a chain come-a-long attached to a metal a-frame. As the ee was exerting downward pressure, the chain slipped on the come-a-long causing the ee to strain his shoulder. The ee was wearing a life preserver during the time of the accident.
