## Narrative

Road Builders Machinery was reinstalling the tracks on the PC600 excavator. When the lifting chain broke it lashed back and hit employee on his hard hat, nose and lip causing a laceration (stitches in his lip). Employee was using a bar to help move the tracks onto the rollers.
