Narrative

EE was tramming roof bolter #10 into position in entry 3 inby crosscut 11. A rock measuring 6'x3'x 2'to 6' thick fell from mine roof striking EE on the head. The rock slid down the left side of his body striking his ankelThis caused contusion to his left ankel and gave him a headache. The rock fell from between the roof bolts at an angle.
