## Narrative

Medical information not received until 6/28/06. Employee was unloading 50 lb bags from bed of pickup. Lifted one bag over side of bed & handed to another employee. As he attempted to lift second bag out of truck heard pop & felt pain in right shoulder extending up his neck. He then lifted bag & carried into lab.
