Narrative

Employee was installing crib blocks in the belt entry. A peice of rock measuring 12"" inches x 6"" inches x 4"" thick fell from mine roof.He suffered a laceration on his forearm. Cause of injury was awareness. Mining equip/systems not a factor. Compliance with rules/regs not a factor. PPE not a factor. Miner attitude could be a factor.
