## Narrative

Employee had released a stone that was wedged between dual tires on a dump truck with a sledge hammer.  After the stone was released and the hammer was on the ground the tire blew up.  The tire did not hit employee, but because the tire was around sand and stone, when the tire blew, sand and small stones flew in the air and hit the employee in the chest, arms and eyes.
