## Narrative

Employee had completed typing the rebar for the foundation shab and was walking across the rebar mat to leave area. As he approached the outside form, a wall tie that had not yet been welded to the rebar mat, caught his boot or pant leg. Causing him to trip and fall. He hit the form with his knee and his elbow landed on asphalt paving.
