Narrative

Employee was unloading 4"" water pipe out of supply shed with another employee. The other employee had lifted one end of the pipe while he reached down to pick up the pipe with his right hand. As he did so he felt a tear just above the elbow of his right upper arm (bicep). Employee went in for out-patient repair of his tendon on 7/13/06.
