Narrative

Employee was changing out a feeder crank disc and while he was using a wrench to break the bolts loose, he felt pain in his left shoulder. An MRI was taken on 7/20/06 and he was told the results on 7/28/06. He was told that the MRI showed some Bursitis in his shoulder and he is now scheduled for Physical Therapy.
