Narrative

Assisting in changing out a section a tailing piping. He was using a 1"" impact wrench to remove bolts from the flange. The impact wrench slipped off of the bolt head. This caused the employee to lose his balance and he used his left hand to catch himself. This resulted in a laceration of his left palm.
