## Narrative

The employee was preparing to hook gauges to a set of compressed gas cylinders. When he cracked the valve on top of the acetylene tank to blow any dust or debri from the valve stem, another employee nearby began using a grinder. A spark from the grinding ignited the acetylene at the valve stem.
