Narrative

Employee was in an upright position roof bolting. He had completed drilling the rib side hole in the roof and was reaching back to get his roof bolt when a piece of rock measuring 25""LX18'WX2""T fell from the roof and struck his right forearm. X rays negative. Employee later fainted making it an emergency.
