Narrative

Employee had just finished helping unload belt structure. He was struck on the head by a rock that knocked his hard hat off. Before he could put his hat back on, a rock approximately 12 inches x 16 inches x 0-2 inches fell and struck him on the back of his head. He received a laceration to the back right side of his head. Four sutures were put in to close the laceration.
