Narrative

Supervisor and another co-worker were carrying a piece of metal. His co-worker dropped his end and caught the victims finger between the metal and the rib. Cause of this injury was lack of communication and enough help. Compliance with rules/reg not a factor, protective equipment not a factor, miner proficiency a factor, mining equip/systems not a factor.
