A Fall of Roof occurred in the First Southwest Mains in the # 3 Entry at x-cut # 7 The Fall was Approx 40'Long x 18' wide x 10' high. The fall originated in an intersection that was supported with 6' fully grouted bolts for primary and 10' cable bolts and cribs for supplemental support. Water was one of the contributing factors associated with the fall.
