## Narrative

After working for a few hours the employee noticed that his right eye seemed to be getting irritated.  He flushed his eye and after it felt better the employee finished the scheduled shift but did not report immediately.  Employee reported the incident to the immediate supervisor and OHN 4 days later because irritation in the eye reoccurred.
