## Narrative

Employee was installing his outside bolt. The bolt bent & he accidentally hit the rotation. The bolt & plate spun cutting his right arm. The investigation revealed that he missed the hole with his bolt & when the bolt bent & he tried to get his hand away from it, he accidentally hit the rotation causing the plate to spin around & strike him in the right forearm lacerating it.
