Narrative

Employee was removing bolt from a suction tube, as he was pulling in his ratchet, he felt a sharp pain in his left shoulder. Medical treatment was sought on 8/05/06 and he was released back work full duty. A follow up and MRI confirmed a tear, employee had surgery on 10/06/06 to repair the shoulder and was taken off work.
