Roof bolter was attempting to put a roof bolt plate over a bolt and it slipped from his grip and the plate fell cutting his arm. Cause of this injury was safety awareness and being in a hurry. Compliance with rules/regs not a factor, protective equip. a factor (Long sleeve shirt), miner proficiency a factor, mining equip/systems a factor.
