Narrative

Unit #2, #3 Face - Employee was bolting face of #3 entry. A piece of rock approximately 30"" square, with thickness of 2 1/2"" tapering off to 1/2"" fell from between bolts. The rock bounced off ATRS, striking left side of lower back and left hip causing abrasions. RTW 11/04/06. Lost 2 Work Days.
