An unintentional roof fall occurred at the 3 East Headgate #2 Entry, 5875'. The fall was approximately 20 feet long, approximately 24 feet wide and approximately 12 feet thick. The fall did not affect ventilation or passage of men. The fall will be cleaned up. Fall #23Q
