## Narrative

Miner was operating LHD on first shift.  As he turned into an intersection to approach the muck pile he ran over a large piece of salt on the ground.  The jolt of hitting the large chunk caused him to bounce out of his seat and contact the fire suppression monitor box located above his head.  The monitoring box cut his scalp about 4"" long and 1/4"" deep.  He received 10 stitches.
