Employee loading a 24' extension ladder into work pick-up, when he felt pain to the left shoulder. Doctor visit on 11-16-06, referred for MRI. MRI done on 11-18-06. No definite determination. Specialist appointment on 11-22-06. Dr. states,""Too early to make recommendation"". Wants to see employee again first week of January 2007. No prescription meds. RTW light duty till then.
